Medical Providers

Dear Physician,
It is your responsibility to determine both the medical need for, and the utilization of, all healthcare services. In addition to the Certificate of Medical Necessity, Medicare requires providers to obtain additional documentation supporting the patient's medical need for a power wheelchair and accessories. This documentation must be available to Medicare upon request. Additional documentation must be corroborated by information in the patient's medical record. This documentation should describe the patient's medical condition, capabilities and limitations necessitating the equipment.
